(Jan 4/12) - After December 31, 2011, members who want an Active Practice Permit who have not completed the 2012 Registration Renewal Application must contact CLPNA for a "Reinstatement Package" at info@clpna.com or 780-484-8886. Individuals without a valid 2012 Practice Permit are not authorized to practice as an LPN in Alberta.
