This tool is designed to guide and assist the LPN with the submission of their CCP Audit for review. This tool provides criteria of acceptable and unacceptable levels of performance for each area of focus.

May 2026 – updates include clarification on the word count requirements for the written portions of the audit form and added additional clarification to the already approved supporting documents section.

March 2026 – updated branding to CLHA, added minimum word count for summary of learning activity description.

August 2024 – minor updates to the verification/proof of learning section including adding podcasts to the list. Preceptorship/mentorship has been removed and will no longer be accepted as proof of learning.
