Overview

A personal directive enables individuals to name someone they trust to make personal decisions for them if they become incapacitated and are unable to make their own personal decisions. Healthcare professionals, including Licensed Practical Nurses (LPNs) may be asked to complete a capacity assessment to establish if a personal directive should be in effect.
